Passport
- — Your passport must be valid for at least 6 months after the date of your visa application.
- — It must also contain at least 2 blank pages.
An identity photo
- — These must be in colour, taken against a plain light background, in 35x45mm format and printed on photo paper (as for your identity card or passport).

An official visa application form

- — Fill in the form in capital letters. Be precise.
- — Write your full name.
- — If applicable, indicate your maiden name.
- — Under the heading «Nationalité», write your current nationality.
- — State your nationality of origin.
- — Write your parents' names.
- — State your family situation.
- — If you are married, write the full name and nationality of your spouse.
- — State your current residence and your telephone number.
- — Write your occupation and the name and address of your employer.
- — Then write the number, the date and place of issue and the expiry date of your passport.
- — Indicate the purpose of your journey (Tourism).
- — State the number of entries of the requested visa.
- — Write the length of stay in Lebanon and the date and place of your first entry into Lebanon.
- — State a person to contact in Lebanon.
- — Write the address and telephone number of your hotel in Lebanon.
- — Sign and date your application form.
A photocopy of the identity card/resident card
- — Applicants who do not hold a Belgian passport must also present a color copy of both sides of their identity card and their residence card in Belgium (if they hold one).
A mission letter
- — Issued by the employer (or by the company in Lebanon), to be attached only if the trip is for professional purposes, specifying the details of the mission (purpose, length of stay, number of entries and coverage of the mission)
- — NB: For minor children, the form must be accompanied by a letter of consent signed by both parents before the consular officer at the embassy, or, failing that, a travel authorization signed at the municipality or before a notary.
